Changes to our National Precertification List (NPL)
This update applies to both our commercial and Medicare members.
Effective February 1, 2023, we will no longer require precertification
for the following antithrombin drugs:
• ATryn® (antithrombin recombinant)
• Thrombate III® (antithrombin III [human])
Effective February 8, 2023, we’ll require precertification for lumbar
total disc arthroplasty.
Velcade Policy update
Precertification of bortezomib (Velcade), for multiple myeloma only, is required of all Aetna participating providers and members in applicable plan designs. For precertification of bortezomib (Velcade), for multiple myeloma only, call (866) 752-7021 (Commercial), or fax (888) 267-3277. For Medicare Part B plans, call (866) 503-0857, or fax (844) 268-7263.
Clotting Factors
Precertification of clotting factors is required of all Aetna participating providers and members in applicable plan designs. For precertification of clotting factors, call Aetna's Special Case Precert Unit at (855) 888-9046.
